What can I expect at a Safety Critical Medical appointment?

A Safety Critical Medical includes a detailed review of your medical history and a full physical exam. This often involves:

  • Blood pressure check
  • Vision and hearing tests (audiometry)
  • Spirometry (lung function)
  • Musculoskeletal screening
  • Mental health and wellbeing assessment
  • Drug and alcohol testing

All assessments are carried out by experienced clinicians following SEQOHS-accredited standards, ensuring your safety and compliance under the Health and Safety at Work Act 1974.

What is a D4 Driver Medical?

A D4 Medical is the medical examination required when applying for or renewing a Group 2 licence (HGV, LGV, PCV, Bus or Lorry). The exam includes:

  • Review of medical history
  • Blood pressure and BMI check
  • Eye test (visual acuity + colour vision)
  • Cardiovascular, neurological and mental health screening
  • Completion of your official DVLA D4 form
  • Appointments typically take 15–20 minutes, and we aim to issue completed forms on the same day.
What documents do I need to bring to my medical?

You must bring:

  • A valid form of photo ID (e.g., driving licence or passport)
  • Your medical records from your NHS GP surgery (printed or electronic summary)
  • Details of any medical conditions, medications, or treatments

Please note: If you do not bring medical records, the assessment cannot be completed, even if you provide a prescription list.

Why do I need to provide a urine sample?

Urine samples are required for:

  • Diabetes screening (glucose check)
  • Drug testing (in safety critical roles or when employer-mandated)

If a drug and alcohol screen is part of your assessment, the sample must be taken at the appointment under controlled conditions.

What kind of Drug and Alcohol Testing do you do?

We offer:

  • Pre-employment drug and alcohol testing
  • Random or unannounced screening
  • Post-incident testing
  • Return-to-work testing

We use UKAS-accredited partner laboratories where applicable and follow a SEQOHS-compliant chain of custody process to ensure all results are legally defensible and accurate.

What questions are asked in a mental health evaluation?

A workplace mental health assessment may explore:

  • Levels of stress, anxiety, or low mood
  • Impact of mental health on job performance
  • Recent changes in behaviour or wellbeing
  • Any work-related pressures or interpersonal issues

Our clinicians provide a clear fitness-for-work opinion and recommend reasonable adjustments where appropriate, in line with SEQOHS and Equality Act 2010 guidance.

Will I receive my completed medical certificate or paperwork on the same day?

In most cases, yes particularly for:

Taxi/PCO Medicals

Safety Critical Medicals

Fit to Work Certificates

Provided that all necessary documentation is supplied at your appointment (especially your medical records), we issue completed paperwork same-day or within 24 hours.
